86-year-old Ross Phillips of rural Soldiers Grove was reported missing by family members, says Richland County Sheriff Darrell Berglin.
Authorities believe Phillips left his home driving a family car Sunday night, some time after 7:00 p.m.
Family members are concerned about Phillips' health. He relies on medication to treat his advanced stage of dementia and Parkinson's Disease.
Phillips is driving a brown 1999 Pontiac Grand Prix, with Wisconsin license plate number 638-CKS.
Phillips is being described as a white male, 5-feet, 4-inches tall, 160-pounds, gray hair and brown eyes.
Anyone with information on Ross Phillips should call their local law enforcement, or the Richland County Sheriff's Department at 608-647-2106.

Richland County Sheriff Darrell Berglin said that foul play isn't suspect in the death of Ross Phillips, 86, of Soldiers Grove. He said that Phillips' body was found in a dry creek ravine after 1 p.m. Thursday. His remains were about 7 to 10 miles from where his car was found abandoned and stuck in the snow on Shannon's Way in Dayton Township. The area is about four miles west of Richland Center, authorities said.
