A passenger train crashed through the end of the line early Monday at Chicago's O'Hare International Airport and barreled up an escalator and stairs, leaving 32 people with non-life threatening injuries, officials said.

... the operator reported that she had worked a lot of overtime before the derailment. The derailment happened just before 3 a.m. Monday at the end of the Chicago Transit Authority's Blue Line at O'Haret. No one suffered life-threatening injuries
 
Thank goodness this didn't happen during the morning rush!
 
Read today the train driver was hired only a year ago
 
NTSB states train driver fell asleep in a previous incident where she overshot a station. Not to worry, CTA says she really didn't fall asleep in that incident. Just closed her eyes for a few minutes.

The operator of the CTA train that jumped the platform and climbed an escalator at O'Hare has admitted dozing off at the controls, and had been admonished last month when she also fell asleep and missed a stop, officials said today.

The new details of the investigation were disclosed this morning by Ted Turpin, the lead investigator of the crash for the National Transportation Safety Board, after officials interviewed the operator.

Not a very good choice to place the escalators there ; imo.
What if there'd been people walking up/down them ?
 
Hmmm I wonder what the working conditions are like?

My friend is a chronically overworked nurse and ran straight into the back of another car due to sheer exhaustion.

All it takes is a moments inattention.

Thankfully no one was hurt.
 
She was off for 17 hours before her 8 hour shift. Hardly a reason to be exhausted.

I disagree. (NOT defending the driver- just making an observation based on my master's thesis research)

The driver works all three shifts in the space of a week by virtue of her 'on call status. She can be / is usually called in at a moment's notice. So she likely got up in the morning- did her daily stuff/ life, and then, just before bedtime, was summoned in to an eight hour shift driving the train.

Fortunately no lives were lost. Let me tell you, fatigue kills.

Its never the individuals fault.
