Patricia Wearn, 73, left her Torbay home at 12.30pm on Monday and has not returned.
The 73-year-old has Alzheimer's and was likely to be confused and disoriented.
Searches of bush areas around Lonely Track Rd, East Coast Rd and Glenvar Rd would continue on Thursday evening.
Members of the Police National Dive Squad searched around 10 waterways and ponds in the area to no avail.
The North Shore police said a survival expert had been consulted.
"There is still hope, and we need everyone in the Torbay, Browns Bay and Long Bay areas to continue to check their properties.
Wearn was described as wearing light blue 3/4 length pants with a patterned top and sandals at the time of her disappearance. She also wears a lanyard around her neck that has her name engraved on it.
