    "WASHINGTON – When federal investigators contacted the Shaw family at their farm in rural West Virginia last winter, they had an unusual request: They wanted to exhume the body of George Nelson Shaw, Sr., who had died several months earlier, to examine his body for foul play.

    The family had thought his death was suspicious. They didn’t understand how in just a few weeks the retired Air Force veteran had gone from bowling to hospice at the local VA medical center.

    Last month, investigators returned to the 111-acre farm to tell the family what they had learned: Shaw had not died of natural causes. He had been killed – one of a string of suspicious deaths at the hospital."

    The ME found 4 injection sites on Shaw's body that tested positive for insulin. Mr. Shaw had no history of diabetes and he had not been prescribed or ordered for insulin injections.

    This is a serial murderer. Nurse?

    U.S. Attorney: 'We're at the beginning of the end' of VA Hospital homicide investigations

    September 2, 2019

    "CLARKSBURG — While federal law enforcement officials have been mostly tight-lipped about the homicide investigations at the Clarksburg Louis A. Johnson VA Medical Center, Bill Powell, U.S. Attorney for the Northern District of West Virginia, shed some light on the cases on Monday.

    Powell, speaking with Hoppy Kercheval on WV MetroNews’ Talkline, confirmed the criminal investigation continues at as quick a pace as possible, with federal agencies devoting major resources to the effort.

    As for why the investigation is taking longer than most people think it should, he explained that the autopsy reports, which have labeled at least two veterans’ deaths as homicides, aren’t enough to gain a conviction.

    And because of the number of people involved with treating patients at hospitals, investigators have to make sure they’ve looked at all potential suspects.

    “We’re waiting on some things ... I don’t want to get too specific. … The best way I can describe it is there’s been a significant amount of work that has been done and we’re not at the beginning of the beginning, we’re at the beginning of the end.”..."

    Manchin meets with VA officials in midst of investigation

    SEP 2, 2019

    "PARKERSBURG — Families fearing a loved one was killed at the Louis A. Johnson VA Medical Center in Clarksburg are frustrated over the lack of information, Sen. Joe Manchin said Friday.

    At least 11 families have contacted his office, wondering if a member of their family is among those dying under suspicious circumstances at the medical center, he said.

    “My heart goes out to the families,” said Manchin, who met with staff and patients Friday at the facility where the deaths last year of at least two veterans have been ruled a homicide by a medical examiner from hypoglycemia, severe low blood sugar, caused by a medically unneeded injection of insulin. The FBI also has asked the family of another patient who died at the facility for permission to exhume the body and perform an autopsy, Manchin said.

    Manchin, speaking to reporters late Friday afternoon after the meeting at the medical center, said they were murdered...."

    this is so creepy
    I'm sure they're already investigating someone and once identified, the stories will start to emerge about how colleagues thought something was wrong, probably this person has been reported to HR already etc.

    "CLARKSBURG — An attorney whose notice of claim brought into the public eye an investigation into suspicious deaths at the Clarksburg VA hospital said all of the cases that he knows about have involved patients who were on Floor 3A....

    The fact that all of the deaths likely happened on the same floor is one link in what could be a circumstantial case on the criminal side.

    O’Dell noted that in each suspicious death, “there’s going to be a time when each of these victims goes south, their care. There are only so many people assigned to work on that floor. So you’re going to have to find someone who matches being there at each of the important intervals.”...

    He noted that the names of patients and their families don’t become public during the notice of claim process. That information only comes out when an actual lawsuit is filed, which doesn’t happen for six or more months after the notice of claim is filed. O’Dell also noted that the families of patients can authorize the release of information ahead of that timeline...."

    Charleston attorney now representing five families regarding Clarksburg VA deaths

    "CLARKSBURG, W.Va. — Charleston attorney Tony O’Dell is now representing five families of veterans who died under suspicious circumstances at the Clarksburg Veterans Affairs facility.

    Eleven deaths are being questioned.

    One of the deceased is 87-year-old John Hallman, who died in June 2018 following insulin injections. O’Dell stressed Hallman’s death has not been ruled a homicide, but it happened on the same floor and under similar circumstances as other veterans.

    The April 2018 deaths of retired Army Sgt. Felix McDermott and George Nelson Shaw, a retired member of the Air Force, at the Louis A. Johnson VA Medical Center have been ruled homicides.

    “We don’t know whether this is just an unbelievably incompetent person, which is hard to believe, or if someone is doing this willfully to people who have served our country,” O’Dell said on MetroNews “Talkline.”...'

    Attorney: Veteran’s family would have wanted autopsy

    "PARKERSBURG — An autopsy would likely have been done had the circumstances been disclosed of how a patient died at the Louis A. Johnson VA Medical Center, said an attorney representing the families of veterans who died at the hospital under suspicious means.

    John Hallman, 87, passed away in June 2018, the day after he was admitted to the medical center for treatment of liver problems and signs of pneumonia. Hallman served in the U.S. Navy during the Korean War.

    The family was never told that the amount of insulin in Hallman’s body had spiked upward at the time of his death, said Tony O’Dell, a Charleston attorney representing Hallman’s family and the families of other veterans dying under suspicious circumstances at the medical center. Attorney Harry Deitzler is co-counsel.

    Hallman was cremated. Had the family been told of the insulin in his body and that there was no reason for its presence, “we would probably have had an autopsy,” O’Dell said.

    Investigators with the independent Office of the Inspector General in the Department of Veterans Affairs in November contacted Debbie Cutler, Hallman’s daughter, and showed the family the amount of insulin in his body spiked just before he died.

    Hallman is the third veteran identified among others dying under suspicious circumstances at the medical center...."

    Fourth man identified in Clarksburg VA hospital deaths

    "PARKERSBURG — An 84-year-old man is the latest to be identified as dying under suspicious circumstances at the veterans hospital in Clarksburg.

    Army veteran Archie Dail Edgell, who lived most of his life in Doddridge County, died March 26, 2018, at the Louis A. Johnson VA Medical Center, said Dino Colombo of Clarksburg, the attorney representing the Edgell family.

    Edgell, drafted into the U.S. Army in 1953, is the fourth person to be identified to have died under suspicious circumstances at the medical center where an investigation is underway by the independent inspector general of the Department of Veterans Affairs.

    The circumstances of Edgell’s passing were similar to the others, Colombo said. Edgell was admitted to the medical center in preparation of being placed in a VA nursing home, but died three days later....

    Allegations of misconduct do not involve current employees at the medical center, Dr. Glenn Snider, director of the facility, has said. The person of interest was removed from the position in Clarksburg, he said...."
