Family members said Isaiah Hernandez, 15, left his home, on Route 11 in Sunapee, with his friends or girlfriend on March 5 and has not been seen since. He was reported missing the next day.
Isaiahs father, Jeffrey Hernandez, saw a Snapchat photo of his son in New York City, police said. Officers sent the information to the New York City Police Departments Manhattan Division.
The teenager is described as white, standing 5 feet 9 inches tall and weighing 170 pounds. He has brown hair and blue eyes.
Police in Sunapee and Newport have followed multiple leads to track down the teenager but have been unsuccessful.
Officers said the teenagers information was put across the Granite State via the New Hampshire Information and Analysis Center.
