Circumstances of Disappearance:
Perkins was last seen at the Towne Center in Baton Rouge, Louisiana just before 10:15 p.m. on June 30, 2012. He was supposed to report there for work, but he never did and has never been heard from again. Perkins suffers from various medical conditions and it is unknown if he had his required medication when he went missing.

At the time of his disappearance, Perkins was driving a red 1996 Chevrolet pickup truck. It was later found abandoned approximately 90 miles away on the roadside on Silver Drive south of Highway 48 in Pike County, Mississippi. There was an unconfirmed sighting of him in Walthall County, Mississippi walking near the Walker's Bridge area. Despite extensive searches conducted, Perkins was never located and his whereabouts remain unknown.
