The suspect had easy access to, and possibly was involved in, fentanyl processing/dealing with his brother. The only explanation for fentanyl in the body of a victim that had suffered extreme head/facial trauma is that powder was deposited into her nose and/or mouth. It's possible her head was slammed into the door frame when she was first forced into the car. Apparently, it took him 4 more minutes to incapacitate/secure her after that. Perhaps he was hoping to cause an overdose, or just subdue her and keep her alive during the assaults.
The description of the victim, especially the heart, in the autopsy report, doesn't show any of the telltale signs of substance abuse of opioids/fentanyl.
